Three Cheers For Sweet Revenge continues the story from I Brought You My Bullets, You Brought Me Your Love where two lovers both died when their hometown was overrun with vampires.
It's told that the male went to hell, and made a deal with the devil so he could have the souls of himself and his lover back in exchange for the souls of evil men.
The album starts off with the funeral of his lover (Helena), as he goes on to collect the souls of a gay criminal (You Know What They Do To Guys Like Us In Prison), a highschool student that went on a shooting rampage (I'm Not Okay (I Promise)), and a man that kills a group of people that attends a wedding party (To The End).
During the soul collecting, the male has his doubts (The Ghost Of You), but in the end he gets his lover back and tells the story of how he did it (I Never Told You What I Do For A Living).
Frontman Gerard Way has said that while the album started with a plot in mind, about half of the album isn't related to the plot as much as it is drawn from his own experiences with love and loss.

The album is a concept album. The storyline continues in their second album Three Cheers for Sweet Revenge.
The story as it goes is about two lovers that break up, but reunite when their hometown is invaded and overrrun by vampires. The female (dubbed as "Helena" on some fansites) ends up being bitten by a vampire and is sent to a hospital where she soon dies, with a spike through her heart from her lover. The male continues to live, but realizes that he cannot continue living without her, and kills himself. However, there are three songs on this album which do not follow the concept above:
"Demolition Lovers" - the male running away with his lover and dying together
"Vampires Will Never Hurt You" - allegedly about the female getting bitten by a vampire, but can be interpreted as the male getting bitten by a vampire
"Early Sunsets Over Monroeville" - based on the storyline of "Dawn Of The Dead", when the female gets bitten by a zombie, not a vampire.
Bullets earned the band a large underground following, but they were not able to emerge into the mainstream until their second album, Three Cheers for Sweet Revenge, was released two years later.
