This album is based on the stress and anxiety issues that Buddy Nielsen, the lead singer of the band, suffered in the years before the album's release. The album's story is told from the point of view of a character's stress and anxiety, including his battles with faith, alcohol, and depression. He eventually takes his own life in the track "All the Best Cowboys Have Daddy Issues"--a track in which leitmotifs from Can't Be Saved are heard. After a brief instrumental interlude ("Negative Space"), the final track, "The Priest and the Matador" tells of the dying character's refusal of spiritual help, and finally, his death.

It is a re-release of the first full-length album by Senses Fail. The special edition of the CD includes 5 bonus tracks, a DVD with live content of the band's tours and both of their music videos, topped with a special holographic cover.

First EP, released on August 13, 2002 on ECA Records. The CD was released with new artwork and two bonus tracks on April 29, 2003 on Drive-Thru Records. The song "Steven" is about a friend of the band's who died in a car accident.
